Service Level Agreement
This forms the basis of the service provided by ACE in supporting a school.
Home/School Agreement
This is the agreement between ACE and the parent/guardian of a pupils being supported by ACE. It also includes a risk assessment for children on a reduced timetable.
Section 19 Referral Form
This form must be completed in full when a school refers to ACE. In addition copies of relevant evidence/reports must be provided.
